Patient Information

For continuity of care and accuracy in filing  insurance claims on your behalf, it is helpful if you  bring the following items to each office visit.

1. Prescription Medications
You may bring all prescription medications you are currently taking in a bag or you may make a list of these medications including exactly how you take them.

2. Medical Records (Especially New Patients)
This is not mandatory, but it is helpful to us if you bring copies of your records and test results (within the last 12 months) from your referring physician.  This will avoid duplication of tests and facilitate optimal care.

3. Insurance Information
Please bring your insurance cards ( both primary and secondary) with you. We will need to make a copy of them for billing purposes.

4. Identification
Please bring your driver's license and social security card.
